Now I'm shocked! I've had friends from Suffolk, Norfolk, Yorkshire, Wales & Australia use it. Maybe it's fallen out of use with the under 40's.

The first time I heard that expression was from an RCAF veteran who was flying Lancasters and stationed at one of the Bomber Command bases in northern England during the war. He, like all flying officers, had a personal batman, one of whose duties was to "knock him up" at pre-dawn when their squadron was due for a sortie. I remember giggling at the reference, until he explained to me that the batman was simply being ordered to knock at his barracks door at some ungodly hour.
